The Physician is responsible for providing care to patients. This includes evaluation, diagnosis, treatment, and documentation. Qualifications MD or DO Board Eligible or Board Certified Family Medicine Physician Eligible for Washington State medical license DEA license and NPI ACLS, PALS, and STABLE certifications (or ability to obtain) Minimum Experience Completion of residency. Exposure to a clinic environment.
