Overview: The Measurement Based Care Coordinator supports consistent, data driven care throughout a client’s treatment journey. You meet with new clients early in treatment, coordinate outcome surveys at set points, and ensure measures are completed on time. You monitor results, flag clinical risks promptly, and work closely with clinical and research teams to support care planning. You are responsible for extracting, organizing, and interpreting assessment data to inform treatment decisions and improve outcomes. Major Responsibilities: 1. Meet with all incoming clients within 48 hours of their arrival and complete the consent for participation and welcome survey. 2. Assign outcome measures to clients depending on the program at three-time points. 3. Coordinate and facilitate the administration of measures for clients at all three-time points and alert PC and clinical management immediately to the presence of suicidal thoughts, workplace conflict, and substance abuse. 4. Extract measure data from KIPU into Excel for scoring and data manipulation. The coordinator will work with the clinical team to ensure clients have completed their assigned survey during times that fit into clients’ schedules.
