Here in Jersey, we have our own Health Care system, separate from the NHS. We are looking to develop and improve our healthcare services towards a modern, fully digitised, and future-proof system, in advance of our new hospital (planned to open in 2029). To do so we need fresh eyes, new ways of working and the next generation of consultants to make our healthcare the best it can be. We aim to provide high quality, safe and effective care to the 110,000 people living on the Island and those who visit. The hospital benefits from a fantastic educational centre with a full SIM lab and dedicated time for all to learn. Aligned to the Wessex Deanery, we support and develop Foundation trainees as well as Clinical Fellows and SAS doctors. We encourage all our consultants to fully engage with the education and training of our future generation of consultants. Jersey is easily accessible from most regional airports in the UK and has good links with NHS tertiary centres especially in Southampton and London, which affords the successful candidates to maintain links within the NHS and the private sector working. The Post The successful applicant will join a team of two Consultant Gastroenterologists, two Staff Grades and a team of specialist Nurses to provide a comprehensive service in Gastroenterology. The successful applicant will ideally have a specialist interest. The successful candidate will have broad experience of inpatient and outpatient gastroenterology and hepatology, fully competent in diagnostic and therapeutic upper and lower gastrointestinal endoscopy, including management of GI bleeding. The applicant will undertake Inpatient specialist reviews where requested and take part in GI-Bleed on-call rota. For newly appointed consultants a mentor will be allocated to support their induction to the post. Medical/Secretaries/Offices There are dedicated medical secretaries supporting the Consultants and their team based within the Endoscopy Unit. The Consultants have a shared office space with IT facilities. Duties Clinical Duties • Jointly develop a service of excellence for gastroenterology which ensures a seamless integration between Jersey and tertiary centres in England. • Participate in the development of the gastroenterology strategy for the island of Jersey including endoscopy services. • To work in close partnership and liaison with Primary Care and other specialist services. • To undertake inpatient specialist reviews for gastroenterology. • To undertake outpatient clinics for both gastroenterology. • To Provide diagnostic and therapeutic upper and lower GI endoscopy. • To provide clinical advice & support to primary care colleagues and other consultants within Health and Care Jersey medical model. • To deliver personalised, evidence informed medical care plans to all patients that are appropriately documented and regularly evaluated. • To provide advice and timely medical reviews and interventions as part of the interdisciplinary model of care. • To participate and attend relevant services’ MDTs. • To liaise with tertiary referral centres as appropriate to provide patient centered care and service improvements. • Maintain good quality up to date medical records including timely discharge letters and summaries. • To ensure appropriate documentation and coding is used to provide consistent audit and monitoring of the service. • To adhere to local prescribing guidelines. • The post holder should be adaptable, flexible and innovative to meet the ongoing challenges within the service. The responsibilities of the role may change from time to time. Leadership and Management • Take part in management and audit activities to support the Care Group Leadership team and Public Health in planning for services and ensuring the service meets key performance indications including excellent patient care. • The post holder will be expected to be fully involved in a ‘Risk Management” programe and support the Governance agenda. • Represent the service at Hospital meetings, as agreed with colleagues. Teaching and Research The department is recognised for junior doctors' training from the Wessex deanery and medical students teaching from Southampton. Active training and teaching are encouraged in this role. It is expected that the post holder will participate in the training of medical staff and that the candidate will ensure that the training and learning needs are identified and facilitated. This includes educational support for the clinical fellows recruited directly to Health & Care Jersey. The post holder should deliver training to a range of professionals across the multi-disciplinary team in medicine to create a service of excellence for Jersey. Opportunities for participating in research are supported when appropriate. On Call Rota The appointee will take part in the GI bleed on-call rota alongside the other two consultant gastroenterologists which will be reflected in the final job plan. The Specialty There are currently two Gastroenterology Consultants, Dr Moses Duku and Dr Mina Hanna, with special interests in advanced diagnostic and therapeutic endoscopy, bowel cancer screening, wireless capsule endoscopy and IBD. They are supported by an IBD nurse specialist, Consultant Radiologist with special interest diagnostic gastroscopy and EUS, two SAS doctors, specialist nurses in alcohol, nutrition nurse, dieticians, a clinical investigations unit which performs 24 hours pH, and a fibro scan service. There is a close working relationship with the GI surgery, radiology (diagnostic and interventional), pathology and oncology departments and we run a weekly GI MDT, IBD and Nutrition MDTs. Our endoscopy unit has highly motivated staff and performs a wide range of endoscopic procedures. The unit currently comprises of two endoscopy suites, fully equipped with all the latest Olympus equipment including AI. The overall endoscopic workload is around 5,000 cases per annum, including gastroscopy, colonoscopy, EMR, EUS, stenting, treatment of gastro-oesophageal varices, insertion of feeding gastrostomy tubes, balloon dilatation, APC and wireless capsule endoscopy. There are dedicated endoscopy lists for bowel cancer screening and symptomatic FIT services. The specialty outpatient services see patients with a wide range of gastroenterology and hepatology conditions. The clinics are divided into General Gastroenterology, Liver and IBD clinics. We have strong links with tertiary centres in the UK and jointly manage complex cases. The directorate is led by a Chief Officer, Tom Walker who leads an executive team including a Director of Clinical Service, Claire Thompson and a Medical Director, Simon West who is also Medical Responsible Officer/Suitable person with the GMC for appraisal and revalidation. There are 4 medical wards, Plemont, Rozel and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit. SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise. On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department. Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area. Care Group Statistics There are 83 medical beds. 12-20 acute medical admissions on average during each 24-hour period on call within a range of approximately 6-24. These include: 25 bedded Acute Assessment Unit (AAU) 5 bedded Enhanced Care Area (ECA) 6 chair Ambulatory Emergency Care (AEC) A well-equipped Clinical Investigations Department providing echocardiography, exercise ECG, respiratory assessment and GI physiology. An endoscopy suite providing a full range of endoscopy and bronchoscopy services. Diabetes Centre providing outpatient care for the diabetic population of Jersey, including retinal photography and specialist nursing support. Renal/Dialysis unit, providing emergency, acute and longer-term renal support as well as management of transplanted patients and assessment and treatment of renal conditions. 6 bedded isolation area for infected or neutropenic patients. Proposed Job Plan All consultants have a 10 PA contract for public work, this is not negotiable. The final job plan will be agreed with the Clinical Director for General and Specialist Medicine, the Chief of Service for Medical Services Care Group, and ratified by the Medical Director. The postholder’s job plan will be reviewed annually and will be appraised according to the current guidance. Provision will be made for a lunch break between morning and afternoon sessions if both are clinical. Qualifications and Skills Essential: Primary Medical Degree Higher Specialty Qualification: MRCP or equivalent Full Registration with the General Medical Council (GMC) with a license to practice (for overseas candidates it is mandatory that a license is obtained prior to appointment) Inclusion on the GMC Specialist Register/Certificate if Completion of Training (CCT) or equivalent in renal or be within 6 months of obtaining this the date of the interview – documentary evidence of this must be provided to support your application.
