The Role We are seeking an experienced Specialist Adult Social Worker (Level 2) to join our established Neuro Rehabilitation team at Caroline House. This is a unique and rewarding opportunity to work in a highly specialist environment supporting adults recovering from acquired or traumatic brain injuries and neurological conditions at a pivotal stage of their rehabilitation journey. You will be based within Colman Hospital in Norwich although you may be required to take some community-based cases for patients with a neurological disorder. Caroline House is a well‑established neuro rehabilitation unit with a strong reputation for delivering high‑quality, person‑centred care. The service supports individuals with complex physical, cognitive and emotional needs, offering intensive rehabilitation within a warm, collaborative multidisciplinary setting. As the Specialist Social Worker within the unit, you will: * Complete comprehensive, strengths‑based assessments under the Care Act 2014. * Apply the Mental Capacity Act, supporting individuals with fluctuating capacity and contributing to best‑interest decisions. * Lead on safe, timely and person‑centred discharge planning, working closely with hospital teams and community services. * Hold a varied caseload of in‑patient rehabilitation cases and some community‑based adults with neurological conditions. * Support families adjusting to sudden or life‑changing injuries, helping them understand the rehabilitation process and future options. * Identify safeguarding concerns and balance risk with autonomy, recovery, and personal outcomes. * Advocate for individuals’ rights, wishes, independence, and dignity. * Work consistently within the principles of Community Led Support (CLS), enabling creative, community‑based solutions. We are looking for a practitioner who brings warmth, curiosity, compassion, and resilience to their practice.
