We’re looking for a US-licensed Registered Nurse (USRN) with strong clinical documentation, communication, and care coordination skills to support case management, report preparation, and client-facing activities . This is a remote, flexible opportunity ideal for nurses who excel at both clinical writing and patient care coordination . Key Responsibilities Clinical Documentation & Reporting Review and summarize medical records, including provider notes, therapy reports, and assessments Prepare accurate and well-structured Case Management (CM) reports for patients with complex conditions (e.g., TBI, SCI, post-MVA) Ensure documentation accuracy, clinical consistency, and HIPAA compliance Utilize Advocate Systems (preferred but not required) Report Components Presenting problem and diagnoses (with ICD-10 codes) Past medical and surgical history Medications, allergies, and treatment summaries Current providers, therapy updates, and patient progress SOAP notes (Subjective, Objective, Assessment, Plan) Comprehensive case management plans, including: Nursing diagnoses SMART goals Evidence-based care strategies Care Coordination & Client Interaction Conduct online appointments/virtual check-ins with clients Assist in developing, implementing, and updating individualized care plans Coordinate with healthcare providers to ensure continuity of care Call provider offices to schedule appointments, follow-ups, and services Monitor patient progress and adjust care plans as needed Serve as a liaison between patients, families, and healthcare teams Qualifications Active USRN license (required) Experience in case management, utilization review, or care coordination Strong clinical writing and documentation skills Knowledge of medical terminology, care planning, and ICD-10 coding (preferred) Excellent verbal and written English communication Ability to manage both independent documentation tasks and live client interactions Strong organizational skills and attention to detail Work Details Remote / Work-from-home Flexible schedule (outcome- and deliverable-focused) Initial trial task: one CM report Opportunity for long-term collaboration based on performance Why Join Us Work with a collaborative remote team where you can combine clinical expertise, patient interaction, and care coordination to improve outcomes for a diverse caseload.
