Description About the Role The Community-Based Counselor provides family-centered mental health counseling, support, crisis intervention, and care coordination services for youth and their families. As part of a multidisciplinary team, Community-Based Counselors deliver trauma-informed, culturally responsive services in homes, schools, and community settings, helping youth and families build stability, strengthen relationships, and address challenges that may impact their well-being and daily functioning. About the Program The San Francisco Wraparound Program partners with the Department of Public Health to support youth and families experiencing significant behavioral, emotional, and mental health challenges. Serving youth who are at high risk for hospitalization, school disruption, placement instability, and other adverse life outcomes, the program provides intensive, strengths-based, and individualized services.
